Under the terms of the Agreement, the undersigned owner of the stud dog and owner(s) of
   the bitch agree as follows:
1. That a stud service will be provided by natural mating, or by artificial insemination
   (AI) upon agreement with owner(s) of bitch.
2. That said stud dog will be current on his ODA hip certification and eye cerf.
3. That prior to breeding of any kind the following clearances and confirmations 
   regarding the bitch will be provided to the stud dog owner.  OFA hip certification
   of “good” or “better,” passing eye CERF, current negative brucellosis and bordetella
   tests, and current standard vaccinations.
4. If requested, the stud dog owner will board bitch to be bred during the time of the
   mating, and will make good faith efforts to provide for a safe and comfortable stay;
   however, the stud dog owner and/or agent will nt be held liable for damages, illness,
   injury or loss during the period the bitch is in their control.  Boarding time will
   be charged $_________ per day.
5. Any veterinary expenses will be covered by the owner(s) of the bitch (i.e., 
   progesterone testing, smears, AI, heath certificates, or any other costs incurred
   from the breeding).
6. Any airport costs/transportation fees are the responsibility of the owner(s) of 
   the bitch, pickup or delivery to the airport is $__________; necessary paperwork
   is the responsibility of the owner(s) of the bitch.
7. The bitch will be given the opportunity of a minimum of two natural breedings; 
   any AI's will be discussed prior to attempting.
8. When using frozen semen, a nonrefundable deposit of $_________(applied to stud fee)
   is required to be paid prior to shipping of semen; all expenses associated with 
   semen, shipping, storage and veterinary services are the responsibility of the 
   owner(s) of the bitch being  bred. Payment  for these services  will be made to the
   storage facility; frozen semen will be released after the above mentioned expenses 
   have been paid.  The stud dog owner is not responsible for frozen semen once it has
   been released from the storage facility.  When using frozen semen it is the 
   responsibility of the owner(s) of the bitch to make sure that he/she is dealing
   with a competent and experienced veterinarian.  The veterinarian selected for
   frozen semen breeding must have extensive experience in surgical or transcervical
   AI procedures and should have onsite storage equipment for the frozen semen.  
   A minimum of three business day's notice is required for securing tank for shipping.
9. This breeding is guaranteed for three(3) or more living puppies at the age of seven
   days.  There will be no return of the initial deposit of there is no pregnancy or 
   live puppy(ies).  However, the bitch (or mutually agreed upon replacement) may be
   sent on her next season for a repeat breeding; balance of the stud fee to be paid 
   as detailed below.
10. For chilled or frozen semen shipments, the owner of the bitch shall pay a 
   nonrefundable deposit of half the stud fee.  For chilled or frozen semen breedings,
   monies must be received before shipment is sent.
11. The stud fee is$__________, or the price of the most expensive puppy, whichever is
   greater; stud dog owner reserves the right to select a puppy lieu of stud fee.  
   Nonrefundable deposit of $_______ must be paid prior to breedings.  The remaining fee
   is due withing 30 days of litter whelp at which time applicable litter registration
   papers will be signed.  All payments to stud dog owner shall be cash, check, or cashier's
   check.
